Register As A Patient?
In order to register with the practice, new patients are required to undergo a new patient health check which will involve a 20-minute review of your personal medical history. Please bring with you your NHS number
if you were previously registered in the UK and any current medication or information that may be helpful to us. For children under six years old we require a record of their immunisation history.
Please ask at reception for the registration forms and the receptionist will then book an appointment for you to see a doctor or a nurse.
